Complete the form with primary and contingent beneficiaries, including full legal names, dates of birth, Social Security numbers, relationships, and percentage allocations totaling 100%. Sign the form, secure witness and spouse signatures as required (spouse signature required in community property states: AZ, CA, ID, LA, NV, NM, TX, WA, and WI), and return it to your representative, mail it to COUNTRY Life Insurance Company / COUNTRY Investors Life Assurance Company, 1701 Towanda Avenue, P.O. Box 2000, Bloomington, IL 61702-2000, or fax it to 1-866-517-7366. If you reside in Ohio, also complete the Beneficiary or Owner Change Request Questionnaire.
