Download the Life Beneficiary Change Form from https://www.countryfinancial.com/content/dam/cfin/pdfs/customer-care/life-beneficiary-change.pdf or request it from your representative. Complete the form with primary and contingent beneficiaries, including full legal names, dates of birth, Social Security numbers, relationships, and percentage allocations totaling 100%. Sign the form, secure witness and spouse signatures as required (spouse signature required in community property states: AZ, CA, ID, LA, NV, NM, TX, WA, and WI), and return it to your representative or mail to COUNTRY Financial, P.O. Box 2020, Bloomington, IL 61702-2020. If you reside in Ohio, also complete the Beneficiary or Owner Change Request Questionnaire.

Yes. COUNTRY Trust Bank, a subsidiary of COUNTRY Financial, provides trust and estate planning services, including investment management, retirement planning, and trust administration. These services can complement life insurance estate planning strategies such as naming a trust as beneficiary or establishing an ILIT. COUNTRY Trust Bank also provides discretionary investment management through the Journey Series managed accounts (CoMPAS, Focus, and Vision). COUNTRY Financial offers three annuity products. The Flexible Premium Deferred Annuity requires a $1,000 minimum initial investment with ongoing payment flexibility (annually, semi-annually, quarterly, or monthly). The Single Premium Deferred Annuity requires a $10,000 lump-sum investment with no additional payments needed. The Single Premium Immediate Annuity also requires a $10,000 lump-sum and begins income payments shortly after purchase with flexible payout frequency options. All annuities grow tax-deferred and are issued by COUNTRY Investors Life Assurance Company, Bloomington, IL. The Journey Series includes three managed account options through COUNTRY Trust Bank and COUNTRY Capital Management Company. CoMPAS invests in professionally selected no-load mutual funds with a $25,000 minimum. Focus provides a higher level of investment management with a suggested minimum of $350,000. Vision is the top-tier service for individuals and businesses with a $1,000,000 minimum, using stocks, bonds, mutual funds, ETFs, CDs, and existing marketable securities. All Journey Series accounts can be used as retirement accounts (Traditional IRA, Roth IRA) or non-retirement accounts. Securities are offered through COUNTRY Capital Management Company, member FINRA/SIPC.
