Download the Life Beneficiary Change Form from https://www.countryfinancial.com/content/dam/cfin/pdfs/customer-care/life-beneficiary-change.pdf or request it from your representative. Complete the form with primary and contingent beneficiaries, including full legal names, dates of birth, Social Security numbers, relationships, and percentage allocations totaling 100%. Sign the form, secure witness and spouse signatures as required (spouse signature required in community property states: AZ, CA, ID, LA, NV, NM, TX, WA, and WI), and return it to your representative, mail it to COUNTRY Life Insurance Company / COUNTRY Investors Life Assurance Company, 1701 Towanda Avenue, P.O. Box 2000, Bloomington, IL 61702-2000, or fax it to 1-866-517-7366. If you reside in Ohio, also complete the Beneficiary or Owner Change Request Questionnaire.
