Mail the notarized form to Citizens Access, PO Box 71209, Philadelphia, PA 19176-6209. Citizens Access supports up to 2 trustees and 2 grantors per trust account.

IRA beneficiary changes require completing the IRA Beneficiary Form, which is available through a Financial Advisor at your local Citizens Bank branch.
