Navigate to Secure Forms

Download the POD (Payable on Death) beneficiary designation form for deposit accounts, or the TOD Agreement for investment accounts

Complete the form with beneficiary details:
- Full legal name
- Date of birth
- Social Security number

Upload the completed form via the Secure Forms page in online banking

Axos will process the beneficiary designation on your account
Required Documents
- Trust name, date established, and tax ID (EIN or SSN)
Special Requirements
- Account must be registered as individual or eligible joint account
- Eligible joint: joint tenants with rights of survivorship or tenants by entirety
- For joint accounts, POD beneficiary receives funds only after death of all account owners
- You may change your designated beneficiary at any time by submitting a new request
- Named beneficiaries override your will
- TOD Agreement for investment accounts is governed by the laws of the State of Nebraska
- IRA accounts use a separate IRA Beneficiary Designation or Change Request form
