Filing a death claim on a Amica policy begins with notifying the Life Claims Department at 1-800-234-5433. Life insurance claims are separate from probate—proceeds transfer directly to named beneficiaries regardless of whether the estate goes through court. The timeline and documentation requirements depend on the policy type and cause of death.

Death claims at Amica can be started through an online portal, which streamlines the initial notification and document upload. Phone and mail options are also available.

Death claim process

Follow these steps to file a death claim with Amica:

Filing a claim

1
Locate the Amica life insurance or annuity policy documents and note the policy number
2
Call Amica Life Claims at 800-234-5433, ext. 89079, and ask for Life Claims (weekdays, 8:30am-5:00pm ET)
3
Provide the policy number, deceased's information, and date and cause of death
4
Gather and submit required documentation:
  • Certified death certificate (request 5-10 copies from the funeral director)
  • Completed claim form (provided by Amica after initial contact)
  • Government-issued ID for the beneficiary or claimant
  • Trust documents and trustee ID if a trust is the beneficiary
5
An Amica representative will guide you through the settlement process and advise on payout options
6
Set up direct deposit for faster payment if available

Required Documents

  • Completed life insurance or annuity claim form (provided by Amica after initial contact)
  • Certified death certificate
  • Government-issued ID for the beneficiary or claimant
  • Policy number and deceased's information
  • Trust documents and trustee ID (if a trust is the beneficiary)
  • Medical records or physician's statement (if the policy is within the contestability period or if requested)

What to know at this institution

Life and annuity death claims run through Amica Life Claims at 800-234-5433, ext. 89079 (weekdays, 8:30 a.m.-5:00 p.m. ET); an Amica account is not required to file, and a claim can also be reported online at amica.com. Amica will require the insured's name and date of death to open the claim and a certified death certificate to settle it; once all requirements are received a settlement is typically processed within 5-10 business days. In some instances Amica may need to obtain medical history information, including doctors' names and treatments, particularly within the two-year contestability period. Amica partners with Empathy to give beneficiaries complimentary, on-demand practical and emotional support (funeral planning, document assistance, grief resources, probate guidance). Life insurance death benefits are generally not subject to income tax for the beneficiary. Property and casualty policies (auto, home, umbrella, marine) have NO death benefit and are not claimed on death: the named insured's policy does not automatically transfer. The executor or successor should notify Amica P&C at 800-242-6422 to keep the property insured during administration, then either transfer the coverage to the heir or trust that receives the property or cancel it once the asset is distributed or sold. Any unearned premium on a cancelled P&C policy is refunded to the estate. Keep all life policies, annuity contracts, wills, and financial instructions in a secure location and review the deceased's bank and card statements for insurance payments to identify every active policy.

Prepare your letter of instruction to Amica

Amica asks for a letter of instruction alongside its claim form. We prepare a transmittal cover letter and the enclosure checklist Amica requires.

Processing timelines at Amica: Once all claim requirements are received, a settlement is typically processed within 5 to 10 business days. Delays may occur if documentation is incomplete, if medical history information is needed, if the policy is within the two-year contestability period, or if the circumstances of death require further investigation. Incomplete documentation is the most common cause of delays—submitting all required documents with the initial claim helps avoid additional processing time.

Documentation required by Amica includes Completed life insurance or annuity claim form (provided by Amica after initial contact), Certified death certificate, and Government-issued ID for the beneficiary or claimant, along with additional paperwork that varies by account type. All death certificates and court documents must be certified copies.

Frequently asked questions

Call Amica Life Claims at 800-234-5433, ext. 89079 (weekdays, 8:30am-5:00pm ET) and ask for Life Claims. Have the policy number and a certified death certificate available. An Amica representative will guide you through the process, explain documentation requirements, and mail you the claim form. You can also report a claim online through the Amica claim center. Submit completed forms and documentation by mail to Amica Scan Center, PO Box 9690, Providence, RI 02940-9690.

Yes. Amica life insurance policies allow you to name a trust as the primary or contingent beneficiary. Contact Amica Life at 800-234-5433 (weekdays, 8:00am-7:00pm ET) to request the beneficiary change form. On the form, provide the full legal trust name exactly as it appears in the trust document, the date the trust was established, and the trust EIN. Include a Certificate of Trust or trust abstract (do not send the full trust document unless requested). Submit the completed form by mail to Amica Life Insurance Company, PO Box 9700, Providence, RI 02940-9700. For estate tax planning, an Irrevocable Life Insurance Trust (ILIT) can own the policy, removing the death benefit from the grantor's taxable estate.

When a home is transferred to a revocable living trust, the homeowners insurance policy should be updated to reflect the trust as the named insured. Contact Amica Mutual (property and casualty) at 800-242-6422 to request the update. Provide the full legal trust name, the date the trust was established, and the trustee information. This ensures the property remains properly insured under the trust ownership. Amica auto and umbrella policies may also need to be updated if the insured has passed away or if ownership has changed.

Amica's Life Claims Department can be reached by phone at 1-800-234-5433 for questions throughout the claims process.

If the deceased held multiple Amica policies, each may require a separate claim or have different documentation requirements. The Life Claims Department can confirm which accounts require individual attention and which can be processed together.
