Complete the form with primary and contingent beneficiary information, including full legal names, dates of birth, relationships, and percentage allocations totaling 100%. Submit the signed form by mail to Amica Life Insurance Company, PO Box 9700, Providence, RI 02940-9700. Only the policy owner can change beneficiary designations.

Amica offers three whole life plans: Whole Life 20 (premiums paid over 20 years, available up to age 80, builds cash value quickly), Whole Life 65 (premiums paid until age 65, available up to age 50, finishes payments before retirement), and Whole Life 100 (premiums paid until age 100, available up to age 80, lowest annual premiums). All three plans offer coverage from $25,000 to over $20 million, locked-in premiums that will not increase, tax-deferred cash value growth, policy loans, a Terminal Illness Rider, and nonforfeiture options.

Yes. Amica term life insurance policies include a conversion option that allows you to convert to whole life coverage without additional medical underwriting. This can be useful if your health has changed since you originally purchased the term policy. The Amica Multi-Year Guaranteed Annuity (MYGA) requires a minimum contribution of $10,000, with subsequent purchase payments permitted only during the first 60 days of the contract and a maximum of $1 million in total purchase payments. The Flexible Premium Deferred Annuity (FPDA) and Single Premium Immediate Annuity (SPIA) each require a minimum contribution of $5,000.
