Insurance proceeds bypass probate entirely and go directly to whoever is listed—regardless of what a will says. Required Documents

  • Trust name, date established, and tax ID (EIN or SSN)

Special Requirements

  • Trusts can be named as primary or contingent beneficiaries
  • Trust beneficiary designation requires the legal name of the trust and date of trust
  • If policy is trust-owned, all trustees must sign the form
  • Certification of Trust may be required if not already on file
  • Designation becomes effective as of its date of execution (signature date)
