With 12 products relevant to estate planning, American Fidelity policyholders need to understand how beneficiary designations control where proceeds go. These designations override a will, so the names on file with American Fidelity are what determine who receives the death benefit.

American Fidelity lets account holders update beneficiary designations online, by mail, and by phone, typically taking 10-15 minutes online; 15-20 minutes by mail. Trust funding is also available, allowing families to name a trust as the policy beneficiary or establish an irrevocable life insurance trust (ILIT).

Complete and submit the Annuity Change of Beneficiary form (ANN-187), available from americanfidelity.com/support/forms/ or by calling 800-662-1113. Return the form by email to annuity-services@americanfidelity.com or mail to Annuity Services Department, P.O. Box 25520, Oklahoma City, OK 73125-0520. You can also fax the form to 405-523-5978. Include each beneficiary's full legal name, Social Security number, date of birth, and relationship to the annuitant.

American Fidelity offers disability income insurance, limited benefit cancer insurance, limited benefit critical illness insurance, limited benefit accident only insurance, Hospital GAP PLAN insurance, and hospital indemnity insurance (AF Hospital Assist) as supplemental products. These are primarily available through employer-sponsored voluntary benefit programs. Supplemental products pay benefits directly to the policyholder during their lifetime and cannot be retitled to a trust. Only life insurance policies (term life, term 100 life, and whole life) support trust beneficiary designations and ILIT ownership transfer. Annuity products support naming a trust as beneficiary.

Download the Change of Beneficiary form (PS-138) from americanfidelity.com/support/forms/ or call 800-662-1113 to request one. Complete the form with each beneficiary's full legal name, address, Social Security number, date of birth, and relationship to the insured. A witness signature is required (the witness must be over 18 and not a beneficiary on the form), or you can provide a copy of the owner's active government-issued ID in lieu of a witness. Submit the completed form by email to beneficiarychanges@americanfidelity.com, by mail to P.O. Box 25523, Oklahoma City, OK 73125, or by fax to 800-620-8915. The change is not effective until approved by American Fidelity.

Yes. A trust can be named as the beneficiary of an American Fidelity annuity using the Annuity Change of Beneficiary form (ANN-187), available at americanfidelity.com/support/forms/. Provide the full legal name of the trust, including the trust date and trustee name, in the beneficiary section. Submit the completed form by email to annuity-services@americanfidelity.com, by mail to Annuity Services Department, P.O. Box 25520, Oklahoma City, OK 73125-0520, or by fax to 405-523-5978. American Fidelity annuity contracts cannot be retitled to a trust while the annuitant is living.

American Fidelity Term Life Insurance provides coverage for a set term with premiums locked in at purchase. American Fidelity Term 100 Life Insurance provides coverage until age 100 with premiums based on issue age that are guaranteed to remain the same for 30 years or to age 85, whichever comes first. After that initial period, premiums may stay the same, increase, or decrease but will never exceed a guaranteed maximum amount. Both products include a life insurance rider with long-term care benefits and can be assigned to an Irrevocable Life Insurance Trust (ILIT).
