Provide each beneficiary's full legal name, relationship, and percentage allocation totaling 100%, and name contingent beneficiaries. To name a trust, use the full legal trust name, date established, trustee names, and trust EIN. Only the policy owner may sign, and the change takes effect once a complete form is received at the AFLIC Home Office (6000 American Parkway, Madison, WI 53783). A beneficiary designation overrides instructions in a will.
