Because Affinity Federal is a membership-based institution, trust retitling must maintain the membership eligibility requirement.   • Full name, date of birth, Social Security number, and relationship
  • Specify percentage allocation if adding multiple beneficiaries

Special Requirements

  • POD designations available on deposit accounts (checking, savings, money market, certificates)
  • IRA beneficiaries require a separate IRA Beneficiary Designation form
  • Joint account owners have automatic right of survivorship
  • Trusts can be named as POD beneficiaries on eligible accounts
  • Online beneficiary changes require third-party cookies and pop-ups to be enabled
