Complete the Non-Financial Change Form (ZA-1025), available for download at zurichamericanlifeinsurance.com or by calling 1-888-634-6780 for life insurance or 1-800-457-9047 for variable annuities. List all primary and contingent beneficiaries with full legal name, date of birth, Social Security number, relationship, and percentage allocation. The new designation cancels all previous designations, so all beneficiaries must be listed even if only changing one. Sign and mail the form to Zurich American Life Insurance Company, 7045 College Boulevard, 8th Floor, Overland Park, KS 66211. The change takes effect on the date signed once received and recorded at the home office.
