Frequently asked questions

Beneficiaries can be updated in the online account at https://www.mutualofomaha.com, or on the Application for LIFE Change of Beneficiary (Form L4237) at https://cdn.mutualofomaha.com/documents/mutualofomaha/pdf/rpd952.pdf, mailed to 3300 Mutual of Omaha Plaza, Omaha, NE 68175 or faxed to Policyowner Services at 402-997-1906. Allocations must be stated in percentages totaling 100 percent, not dollar amounts, and a separate form is required for each policy or rider. A trust is entered using the form's Other Trust Arrangements wording, naming the trustee, any successor in trust, and the date of the trust agreement. Massachusetts residents must have a disinterested adult witness the change, and no change binds the company until it is received and recorded at the home office. Group life beneficiary changes go through the employer.
