Log in to your MyTransamerica account to initiate a beneficiary change online, or call 1-800-797-2643 (life insurance) to request the Beneficiary Change Form (BEN-CCC). Complete the form with primary and contingent beneficiary details, sign it, and return it by mail. You can also download the form from the MyTransamerica service forms page.

Contact Transamerica at 1-800-525-6205 (annuities) or download the Annuity Policy Change Form from myta.transamerica.com/public/service_forms.aspx. Complete the form naming primary and contingent beneficiaries. For trusts, the trust must be listed as the Sole Primary Beneficiary. Sign and mail the form to Transamerica.

Yes. Permanent life insurance (whole life, universal life, IUL) and term life insurance can be assigned to an Irrevocable Life Insurance Trust (ILIT). Complete the Transfer of Ownership Request Form (OWN-CCC) and the Trustee Certification form (TRSTCERT). Include the trust title page and signature page. The trustee signature must include trust name and date.
