For group policies, sign in to your Sun Life account at sunlife.com/us to update beneficiaries online, contact your employer HR department, or call Sun Life Member Services at 800-247-6875 (Monday-Friday, 8am-8pm ET). You can also obtain the Beneficiary Designation form (GLFM-1233) and mail it to Sun Life, P.O. Box 219572, Kansas City, MO 64121. Members on Waiver of Premium must submit the form directly to Sun Life. For individual life policies, call 800-862-6266 to request the Change of Beneficiary form. For term policies with an SC or SU prefix, call 877-431-7379 or email admin@sunlifeterm.com.

Yes. Sun Life group life and AD&D plans offer conversion and portability options when you leave employment. Conversion allows you to convert your group insurance to an individual Sun Life life policy without additional medical underwriting. Portability allows you to continue group term coverage under a Sun Life portability policy. You must apply within 31 days of the date your coverage was terminated or reduced. You may elect conversion or portability, but not both for the same coverage. Contact Sun Life at 800-247-6875 for details.
