You can add or update primary and contingent beneficiaries with percentage allocations totaling 100%. You can also download the Beneficiary Change Request form (Form SVC-102-PL) and mail it to Protective Life Insurance Company, P.O. Box 12687, Birmingham, AL 35202-6687.
