Physicians Mutual offers 10 insurance products that play a central role in estate planning. Because policy proceeds pass directly to named beneficiaries—bypassing probate entirely—keeping designations current is one of the highest-impact steps policyholders can take.

Physicians Mutual lets account holders update beneficiary designations online, by mail, and by phone, typically taking 15-20 minutes to complete; 2-4 weeks for processing. Trust funding is also available, allowing families to name a trust as the policy beneficiary or establish an irrevocable life insurance trust (ILIT).

Beneficiary changes must be submitted in writing. You can log in to My Account at https://www.physiciansmutual.com/myaccount/login and complete the form online, download the Change of Beneficiary form from the Customer Center at https://www.physiciansmutual.com/web/customer-center/forms, or call 800-228-9100 to request the form. Complete the form with your primary and contingent beneficiaries and their percentage allocations. Mail the completed form to Physicians Mutual Insurance Company, P.O. Box 2018, Omaha, NE 68103-2018, or fax to 402-633-1088.

Yes. Physicians Mutual offers four individual dental insurance plans: Premier (Schedule 4), Preferred (Schedule 3), Standard (Schedule 2), and Economy (Schedule 1). All plans pay 100% for preventive (Type I) services when using an in-network provider. The Premier plan covers over 400 procedures and pays 70% of the maximum allowable charge for basic and major services. The Preferred, Standard, and Economy plans each cover 350+ procedures and pay a set dollar amount for basic and major services. No deductible is required and there is no annual maximum on cash benefits. Network of over 600,000 provider locations. Optional vision and hearing benefits can be added. Dental insurance cannot be retitled to a trust.

Physicians Mutual Insurance Company is the parent company that markets and sells the products. Physicians Life Insurance Company is a subsidiary that issues life insurance policies and Medicare Supplement insurance. When you purchase a term life, whole life, or Medicare Supplement policy through Physicians Mutual, the policy is legally issued by Physicians Life Insurance Company. Both companies are headquartered in Omaha, Nebraska, and share the same customer service line at 800-228-9100.

Physicians Mutual offers Medicare Supplement (Medigap) plans including Plan A, Plan F, Plan G, High-Deductible Plan G, Innovative Plan G, and Plan N. High-Deductible Plan G has a $2,700 calendar year deductible for 2026. Innovative Plan G features lower premiums with a plan deductible for the first three calendar years that goes away after year three while premiums remain lower. Plan N pays 100% of Part B coinsurance except for a copayment of up to $20 for some office visits and up to $50 for emergency room visits not resulting in inpatient admission. Medicare Supplement plans are available in approximately 30 states and Washington, D.C., with availability varying by state. Plans are issued by Physicians Life Insurance Company.
