For Pacific Life policyholders, estate planning comes down to one critical step: making sure the right beneficiaries are named on every policy. Insurance proceeds bypass probate entirely and go directly to whoever is listed—regardless of what a will says. When estate tax planning or controlled distributions are a factor, a trust can be named as beneficiary instead.

Required Documents

  • Trust name, date established, and tax ID (EIN or SSN)

Special Requirements

  • Trusts, estates, and charities can be named as beneficiaries
  • Naming a trust as beneficiary or owner requires an extra form: 15-47910 for life insurance, 13077-25A for annuities. Changes naming a trust without it are not processed.
  • On an annuity, changing the OWNER to a trust automatically changes the BENEFICIARY to that trust as well (per form W25065-23B). Complete the beneficiary section if you want a different result.
  • A change of annuity owner may be a taxable event, may affect optional living/death benefit riders, and cancels any scheduled withdrawal program
  • A life insurance ownership change invalidates any existing telephone or electronic authorization on the policy
  • PL Promise policies are serviced by Lynchburg Operations and use their own forms, P.O. Box, and fax
  • New York contracts are issued by Pacific Life & Annuity Company and use different forms, addresses, fax numbers, and phone numbers
  • Primary and contingent beneficiaries are supported
  • If no beneficiary is designated, proceeds may be payable to the owner's estate
