The process depends on the product type. For life insurance, download the Ownership, Beneficiary, or Name Change Request Booklet (BK-10349) from https://www.pacificlife.com/home/individuals/life-insurance/forms.html, or form 15-50626 for PL Promise products. For annuities, download the Annuity Contract/Beneficiary Change Request form (W25065-23B) from https://www.pacificlife.com/home/individuals/annuities/forms.html. Complete the form with your beneficiary designations and submit by mail or fax. Call (800) 347-7787 for life insurance or (800) 722-4448 for annuities with questions.

For non-qualified annuities, naming a trust as beneficiary is generally preferred over trust ownership because a non-natural owner (such as a trust) may lose the benefit of tax deferral on the annuity. For qualified annuities (IRAs), trust ownership is not applicable. Consult with a tax advisor or estate attorney before naming a trust as owner of an annuity. The Trustee Certification and Disclosure form (13077-25A) is required when a trust is named as beneficiary or owner.
