For life insurance, download the Ownership, Beneficiary, or Name Change Request Booklet (BK-10349) from https://www.pacificlife.com/home/individuals/life-insurance/forms.html, or form 15-50626 for PL Promise products. For annuities, download the Annuity Contract/Beneficiary Change Request form (W25065-23B) from https://www.pacificlife.com/home/individuals/annuities/forms.html. Complete the form with your beneficiary designations and submit by mail or fax. Call (800) 347-7787 for life insurance or (800) 722-4448 for annuities with questions.

Pacific Life requires the Trustee Certification and Disclosure form whenever a trust is named as beneficiary or owner so the carrier can document trustee authority and the trust's tax classification without holding the full trust instrument on file. For annuities, this is form 13077-25A (https://www.pacificlife.com/content/dam/paclife/rsd/annuities/public/forms/maintenance/13077.pdf), submitted with the beneficiary change form W25065-23B. For life insurance, the equivalent is the Trust Information form 15-46147 (https://life.myaccount.pacificlife.com/policyowner/ServiceForms/GetFile/15-46147), submitted with BK-10349. Without these forms, beneficiary or ownership changes naming a trust will be rejected.
