Pacific Life documents trustee authority on its own certification form rather than keeping the full trust instrument on file, and it will not process a trust designation without it. For life insurance the required form is the Trust Certification for In-Force Policies (15-47910) -- named on both BK-10349 and the PL Promise beneficiary form 15-50626, which state it must be completed when a trust becomes the new policyowner or the new primary beneficiary. Form 15-47910 is not posted as a public download; request it from the Life Insurance Division at (800) 347-7787. For annuities the equivalent is the Trustee Certification and Disclosure (13077-25A), a public download at https://www.pacificlife.com/content/dam/paclife/rsd/annuities/public/forms/maintenance/13077.pdf, filed with the beneficiary change form W25065-23B. Form 15-46147 (Trust Information) is a separate item: it is what a trust completes to become the OWNER of a PL Promise policy.
