How to name beneficiaries and fund a trust at Ohio National

Covers 12 insurance accounts — beneficiaries can be updated by phone

Brand change

The Ohio National Life Insurance Company is now AuguStar Life Insurance Company, a Constellation Insurance company. The rebrand was announced July 17, 2023 (legal name change effective Q4 2023) following Constellation's sponsored demutualization acquisition, completed March 2022. Existing Ohio National policies and annuity contracts remain in force under the same legal entity. ohionational.com no longer serves a homepage — it redirects to welcometoaugustar.com — and all claims, forms, and servicing now run through AuguStar Financial (augustarfinancial.com). Effective July 2023.

The procedures below reflect Ohio National's accounts. Account servicing may transfer as the change takes effect.

Estate planning with Ohio National policies centers on beneficiary designations—the single most important step for ensuring life insurance proceeds and annuity benefits reach the intended recipients without probate involvement. Unlike bank or brokerage accounts, insurance products are not retitled into trusts; instead, trusts are named as beneficiaries when estate tax planning or controlled distributions are needed.

Ohio National has 12 product types, and the estate transfer rules differ across them. Some support beneficiary designations, some can be retitled into a trust, and others will require probate if nothing is set up. Each is covered below.

Required Documents

  • Trust name, date established, and tax ID (EIN or SSN)

Special Requirements

  • The Beneficiary Change Request must be RECEIVED by AuguStar within 90 days of the signature date.
  • Both primary and contingent beneficiaries must be restated on the form even when only one tier is changing.
  • If percentages are left blank, AuguStar pays equal shares.
  • Name the trust, not the trustee. AuguStar will not pay an individual trustee and will not administer restrictions inside a trust.
  • Contracts owned by a retirement plan or by a custodian CANNOT change the beneficiary at AuguStar — the plan or the custodian must remain the primary beneficiary, and the change is made with the plan administrator or custodian.
  • A Joint GLWB rider requires the participating spouse to be either the sole primary beneficiary or a joint owner. Naming someone else as primary beneficiary TERMINATES the joint benefits of the rider while the full joint rider charge keeps being assessed — AuguStar will not process such a change without a signed acknowledgement of that consequence.
  • Life and disability servicing forms are login-gated at augustarfinancial.com and are not available as public downloads.
