Request a POD (Payable on Death) beneficiary designation form for deposit accounts, or a beneficiary designation form for IRAs

Provide beneficiary details:
- Full legal name
- Date of birth
- Social Security number
- Primary or contingent designation
- Percentage allocation if naming multiple beneficiaries

Complete and sign the designation form with a banker

Receive confirmation that the beneficiary designation has been added to your account
