Download the Formal Trust Account Application and Trustee Certification form from discover.com/online-banking/forms-docs/. All trustee signatures must be notarized, and each trustee must complete a separate notary acknowledgment. Trust documents must permit co-trustees to act individually. Submit documents by uploading at discover.com/online-banking/secure-document-upload/ (select "Trust Application & Trust Certification Form"), or mail to Discover, PO Box 30416, Salt Lake City, UT 84130-0416. Discover reviews within 7 business days.

Without POD beneficiaries, your deposit accounts will go through probate and be distributed according to your will or, if you have no will, according to state intestacy laws. An estate representative will need to provide Letters Testamentary or Letters of Administration, and funds will be made payable to the Estate.
