Frequently asked questions

Complete the IRA Change Information Form, available through Citi Online or by calling Retirement Plan Services at 1-800-695-5911. The information you provide on the form replaces any prior beneficiary designation. Changes must be in writing and are effective when received by Citibank. Mail completed forms to Retirement Plan Services, PO Box 790012, St. Louis, MO 63179 (or for overnight delivery: Citibank N.A., 1000 Technology Drive, MS 100 - RPS Retirement Plan Services, O'Fallon, MO 63368). Naming a trust as IRA beneficiary requires providing the trust's Tax ID Number and a copy of the trust instrument to Retirement Plan Services.
