For Bankers Fidelity policyholders, estate planning comes down to one critical step: making sure the right beneficiaries are named on every policy. Insurance proceeds bypass probate entirely and go directly to whoever is listed—regardless of what a will says. When estate tax planning or controlled distributions are a factor, a trust can be named as beneficiary instead.

Bankers Fidelity has 8 policies with different estate transfer rules. Here is how beneficiary designations, trust ownership, and probate apply to each one.

Call Bankers Fidelity Policyholder Services at 866-458-7500, or email bflphs@bflic.com (the Home Office contact sheet also lists customercare@bflic.com)
2
Request the Change Form (B 0136 CF) for your policy, or download it at https://bankersfidelity.com/wp-content/uploads/2025/03/B-0136-CF-2-13-.pdf
3
Provide your policy number and personal information for verification
4
Complete Section 3, Change of Beneficiary, and return the form by mail or fax

Required Documents

  • Trust name, date established, and tax ID (EIN or SSN)

Special Requirements

  • All beneficiary changes must be submitted in writing on the Change Form (B 0136 CF); changes cannot be made online
  • The same form also handles name changes (Section 1), ownership transfers (Section 2), and a request for a certificate of insurance or duplicate policy when the original cannot be found (Section 4); only the sections checked apply
  • The signature block provides lines for the Policy Owner, a witness, and an assignee officer (used when a corporate assignee records an assignment)
  • A beneficiary signature is not required to change a beneficiary, but the Policy Owner must sign
  • Beneficiary designations override instructions in a will
  • Children under 18 cannot receive benefits directly; if a minor is named, the claim requires probate-court guardianship documents before payment
