Otherwise, download the Beneficiary Change Request form from athene.com/support/service-forms, complete it with your primary and contingent beneficiary information, and submit by mail or fax to 866-709-3922. You can also call 888-266-8489 to request the form. If the current beneficiary is irrevocable, that beneficiary must approve the change.
Yes. To name a trust as beneficiary, complete the Beneficiary Change Request form along with Athene's Trust Verification Form (both at athene.com/support/service-forms). The Trust Verification Form collects the trust name, trust date, EIN, and trustee information. Retitling a non-qualified annuity to trust ownership may trigger adverse tax consequences, so naming the trust as beneficiary is the standard approach for annuity estate planning. Consult a tax or estate planning attorney before retitling.
