View details →You can update your beneficiary online through your American Fidelity account, by calling 800-662-1113, or by downloading the Change of Beneficiary form from americanfidelity.com/support/forms/. Complete the form with each beneficiary's full legal name, address, Social Security number, date of birth, and relationship to the insured. A witness signature is required, though a copy of the owner's active government-issued ID can be provided instead. If the policy has joint ownership, all owners must sign. The beneficiary change is not effective until approved by American Fidelity. Mail the form to P.O. Box 25160, Oklahoma City, OK 73125-0160, or fax to 800-620-8915.
Yes. Beneficiary designations on American Fidelity insurance and annuity policies take precedence over instructions in a will. The new beneficiary designation replaces all previous designations, so be sure to rename all beneficiaries who should receive proceeds when making changes. Review and update beneficiary designations regularly, especially after major life events such as marriage, divorce, or the birth of a child.
Complete and submit the Annuity Change of Beneficiary form (ANN-187), available from americanfidelity.com/support/forms/ or by calling 800-662-1113. Return the form by email to annuity-services@americanfidelity.com or mail to Annuity Services Department, P.O. Box 25520, Oklahoma City, OK 73125-0520. You can also fax the form to 405-523-5978. Include each beneficiary's full legal name, Social Security number, date of birth, and relationship to the annuitant.
American Fidelity offers disability income insurance, limited benefit cancer insurance, limited benefit critical illness insurance, limited benefit accident only insurance, Hospital GAP PLAN insurance, and hospital indemnity insurance (AF Hospital Assist) as supplemental products. These are primarily available through employer-sponsored voluntary benefit programs. Supplemental products pay benefits directly to the policyholder during their lifetime and cannot be retitled to a trust. Only life insurance policies (term life, term 100 life, and whole life) support trust beneficiary designations and ILIT ownership transfer. Annuity products support naming a trust as beneficiary.
