View details →Non-qualified annuity contracts can be transferred to trust ownership using the Transfer of Ownership Request form (SFAL0013). An Entity Customer Information form and certification of trust are also required. Note that transferring ownership of a non-qualified annuity to a trust may result in a taxable event; Allianz will send an "Awareness of Taxation" letter that must be signed and returned before the transfer is processed. Ownership change to a corporation is not allowed for fixed annuity contracts.
You can update beneficiaries online by logging in to your account at allianzlife.com and navigating to "Personal Accounts." Alternatively, download form S2263 for fixed annuities and life insurance (contracts beginning with 1/2/3/4/5/7) or form USA-383 for variable annuities (contracts beginning with A/D/G). Submit signed forms by mail, fax, or email. Trusts can be named as primary or contingent beneficiaries by providing the trust name, trust date, and EIN.
