If spousal consent is required, you will need to download a beneficiary form, obtain a notarized spousal signature, and submit it to your plan administrator. You can also call ADP Participant Services at 800-695-7526 to request a form.
mykplan.com is the participant portal for ADP retirement plan accounts. You can view your account balance, manage investments, designate beneficiaries, change contributions, transfer funds, model loans, and request withdrawal forms. The portal is available online and through the ADP mobile app.
For qualified retirement plans (401(k), 403(b), defined benefit, profit sharing), federal law (ERISA) requires spousal consent with a notarized signature if you name someone other than your spouse as the primary beneficiary. If spousal consent is needed, you must download a paper beneficiary form, obtain the notarized spousal signature, and submit it to your plan administrator. SIMPLE IRAs and SEP IRAs generally do not require spousal consent.
