You can change your beneficiary by logging in to your Symetra My Account at profile.symetra.com and completing the online form, or by downloading the Beneficiary Change Request form from symetra.com and mailing or faxing it. You can also call 1-800-796-3872 (Monday-Friday, 7:00am-4:00pm PT) to request the form. Provide each beneficiary's full legal name, date of birth, Social Security number, relationship, and percentage allocation (totaling 100%). Completing the form revokes all prior beneficiary designations. In community property states, spousal consent is required when changing the primary beneficiary from a spouse.
