  • Request for a beneficiary change on your annuity contract or life insurance policy

A representative will mail you the appropriate Non-Financial Change Form

Required Documents

  • Trust name, date established, and tax ID (EIN or SSN)

Special Requirements

  • Beneficiary changes must be submitted in writing via the Non-Financial Change Form
  • Separate forms are available for fixed, fixed index, and variable annuity products
  • Only the contract or policy owner can change beneficiary designations
  • Beneficiary designations override instructions in a will
  • Multiple primary and contingent beneficiaries can be designated
  • Percentage allocations must total 100% for each beneficiary tier
  • For qualified retirement plans (403(b), 457), spousal consent may be required for non-spouse beneficiaries
  • Forms can also be faxed to 785-368-1772 (annuity products)
