Complete SBLI's Ownership and/or Beneficiary Change Request form (Form K-109CT), available at my.sbli.com or by calling 800-694-7254. You can submit it online through the Customer Hub, by email to records@sbli.com, by fax to 781-994-4240, or by mail to SBLI, P.O. Box 4048, Woburn, MA 01801-4048. You must include proof of identity (a copy of your driver's license or other government-issued ID). If an irrevocable beneficiary was previously named, that individual must consent in writing on the form before SBLI will process the change.

No. SBLI is licensed in 49 states and the District of Columbia but is not available in New York. SBLI is not affiliated with SBLI USA Life Insurance Company, Inc., a separate New York-licensed insurer that was acquired by Prosperity Life Group and rebranded as Shelterpoint Life. New York residents who want to fund an ILIT or name a trust as beneficiary on a similar mutual life product should look to a different carrier.
