Download the Contract Information Change Form (https://oceanviewlife.com/wp-content/uploads/2023/10/OVLAC-Contract-Service-Change-Form-REV-02-21-1.pdf) or call 888-295-3815 (option 2, then option 1) to have one sent.

Complete Section 1 (owner and contract information) and Section 5 (beneficiary change). Completing Section 5 revokes all prior primary and contingent designations — restate every beneficiary you intend to keep.

Attach the Trust Verification Form if a trust is being named, and the Additional Beneficiary Form if you run out of lines.

Special Requirements
- Completing the beneficiary section of the Contract Information Change Form revokes ALL prior primary and contingent beneficiary designations — restate the ones you want to keep.
- Percentages must total 100% for primary beneficiaries and 100% for contingent beneficiaries.
- Naming a trust as beneficiary requires the Trust Verification Form in addition to the change form.
- An Oceanview annuity is owner-driven: the contract owner controls the designation, and the beneficiary designation — not the will — controls who receives the death benefit.
