- Trust name, date established, and tax ID (EIN or SSN)
Special Requirements
- Completing the beneficiary section of the Contract Information Change Form revokes ALL prior primary and contingent beneficiary designations — restate the ones you want to keep.
- Percentages must total 100% for primary beneficiaries and 100% for contingent beneficiaries.
- Naming a trust as beneficiary requires the Trust Verification Form in addition to the change form.
- An Oceanview annuity is owner-driven: the contract owner controls the designation, and the beneficiary designation — not the will — controls who receives the death benefit.
