To open a personal trust account, complete the Account Application for Individual and Personal Trusts online at https://www.secureaccountview.com/BFWeb/clients/dodgeandcox/nasu or download the PDF from the Forms and Guides page. Provide the trust name, date established, trustee information, and tax identification number (grantor SSN for revocable trusts or EIN for irrevocable trusts). Include a Certification of Trust or full trust agreement and government-issued ID for all trustees. To retitle an existing account to a trust, complete the Change of Ownership Form.
