Yes. Use the Policy Service Form and complete the Ownership Designation section on Page 1 and the Signature sections on Page 2, along with the included W-9 for the new owner (the trust). Provide the full legal trust name, trustee name, and trust EIN. The trustee of the ILIT should be the owner, and the insured cannot serve as trustee of an ILIT that owns a policy on their life. If an existing policy is transferred to an ILIT, the three-year lookback rule under IRC Section 2035 applies for estate tax purposes. Cincinnati Life does not establish ILITs; consult an estate attorney to create one.

Yes. Ownership of a Cincinnati Life deferred annuity can be changed to a trust using the Ownership Designation section of the Policy Service Form. Provide the full legal trust name, trustee name, date established, and trust EIN. Complete the W-9 Taxpayer ID Certification for the new owner (the trust). Note that changing annuity ownership may have tax consequences; consult a tax advisor before making changes. Contact Life Policy Services at 800-783-4479 or your independent agent for assistance.

Cincinnati Life requires that the Policy Service Form be signed with qualifying witnesses. A qualifying witness is any adult other than the insured, designated beneficiary, or policyowner. This applies to beneficiary changes, ownership transfers, and other policy service requests. The witness requirement helps protect policyowners by adding a layer of verification to changes. Complete the Signature section on Page 2 of the Policy Service Form with the required witness signatures before submitting.
