To name a trust, complete the Life and Annuity Policy Information form (available at my.bankerslife.com/forms or through your Bankers Life agent), provide the full legal trust name, the date the trust was established, and the trust EIN, then mail it to PO Box 1937, Carmel, IN 46082-1937, or fax it to 312-396-6629. Policies underwritten by Bankers Conseco Life Insurance Company of New York require that company's own form instead.
Yes. Non-qualified annuities (those purchased with after-tax dollars, not held inside an IRA or employer plan) can be owned by a trust. Contact your Bankers Life agent to request the appropriate ownership change forms. Qualified annuities held inside an IRA cannot be retitled to a trust, but a trust can be named as the IRA beneficiary.
