Retitling a checking, savings, or CD account to a revocable trust requires an in-branch visit with the full trust agreement or a Certification of Trust (preferred for privacy) and government-issued photo ID for each trustee. Any existing POD designation must be removed before the account is retitled to the trust, since trust-held accounts generally do not carry POD beneficiaries.
