You'll need to create an amendment to your trust document that removes your ex-spouse and updates beneficiary designations. The amendment must be signed, dated, and notarized according to your state's requirements. Don't forget to also update beneficiary designations on retirement accounts, life insurance policies, and bank accounts, as these typically override what's written in your trust.
