1Visit any Provident Bank branch with your government-issued photo ID
2Request to add a Payable on Death (POD) beneficiary to your account
3Provide beneficiary details: full legal name and identifying information
4If naming multiple beneficiaries, specify percentage allocation
5Review and sign the beneficiary designation form

Required Documents

  • Trust name, date established, and tax ID (EIN or SSN)

Special Requirements

  • POD beneficiary does not acquire an interest in the account until after the death of the account owner or last surviving joint owner
  • Account owner may close the account, remove or add POD beneficiaries, change account type or ownership, and withdraw funds at any time
  • Named POD beneficiaries override your will
  • State law in the state where the account is domiciled may impose additional requirements
