1Visit any Origin Bank banking center with government-issued photo ID

2Request to add or update a POD (Payable on Death) beneficiary designation on your deposit account

3Provide beneficiary information: full legal name, date of birth, Social Security number, and relationship

4Specify primary and contingent beneficiaries with percentage allocations

5For a trust as beneficiary: provide trust name, date of trust, trustee name, and trust tax identification number

6Sign the beneficiary designation form
